The Cape Girardeau County Public Health Center is the Local Registrar for all births and deaths that occur in Cape Girardeau County and can issue certified copies of birth certificates for anyone born in the State of Missouri after 1920. We can issue certified copies of death certificates for anyone dying in the State of Missouri after 1980. Certified copies of other years of birth or death can be obtained from the Missouri Department of Health and Senior Services.

Required items are the Application (which can be printed from above link or they are provided at the Health Department) and Proper Identification (driver’s license is preferred). The certificate will be printed while you are in the department.  We accept cash, local check and debit/credit.  If you use a card a two dollar fee will be added. In-Person Requests:

The Cape Girardeau County Public Health Center can provide birth or death certificates for the State of Missouri between 8:30 and 4:00 Monday thru Friday.  No appointment is necessary. 

Mail-In Requests:

Requests for records in the State of Missouri must be NOTARIZED by an acceptable notary public and mailed with a MONEY ORDER and SELF-ADDRESSED STAMPED RETURN ENVELOPE.
